Please note that a separate request will have to be filled out for each individual student. Form links are also separated by Building level. Please be sure you are on the correct form for the student you are requesting information or your request may be delayed.

We value student safety and privacy. We will verify school and requestor information before sending records.

You will be sent an enrollment confirmation form within 48 hours of submission. Please fill and send back to us for our reporting.
